Description

Case Manager – a one‑on‑one role focused on promoting independence and quality of life for psychiatrically disabled residents in a treatment environment.

Key Responsibilities

Build Meaningful Relationships

You’ll work directly with residents to understand their individual needs, strengths, and goals. Through compassionate assessment and ongoing collaboration, you’ll help create personalized pathways toward greater independence and wellbeing.

Teach and Empower

From household management to nutrition to daily living skills, you’ll be both teacher and coach. You’ll help residents build the practical life skills they need while encouraging their growth every step of the way. You’ll monitor medications, ensure household safety, and create structures that support healing.

Be an Advocate and Bridge

You’ll serve as a vital connection between residents and the broader community working with families, healthcare providers, and support systems to ensure holistic care. You’ll advocate for your clients and help them navigate complex systems with dignity.

Collaborate and Grow

As part of a multidisciplinary team, you’ll contribute to treatment planning, participate in quality improvement initiatives, and engage in ongoing training. We’re committed to your professional development because when you grow, our entire community benefits.

Required qualifications:

  • Associate’s degree in a Human Services field (Bachelor’s preferred)
  • Valid driver’s license and good driving record
  • Ability to perform physical tasks including lifting up to 25 pounds, standing, walking, and assisting with daily living activities

Experience working with psychiatrically disabled individuals is valued but not required—we’re committed to supporting your development in this field.
