• Support patient recovery and enable them to stay at home.
• Educate patients and families on disease management.
• Case‑manage and provide clinical oversight to an interdisciplinary team.

• RN license in the state where you will work.
• Minimum two years of registered‑nurse experience; home‑health experience preferred.
• Valid driver’s license.
• Current CPR and BCLS certification.
