About This Role

The Licensed Field Care Coordinator is responsible for coordinating member care needs for medically complex individuals requiring intensive medical and psychosocial support. This role is ideal for those who are passionate about improving healthcare access and outcomes for members.

What You'll Do
  • Apply clinical knowledge to effect efficient case management for Medicare/Medicaid benefits
  • Incorporate evidence-based clinical practices into care coordination activities
  • Deliver member-centered, individualized self-management support and patient education
  • Ensure appropriate utilization and consistent application of health benefits
  • Conduct telephonic and face‑to‑face assessments according to Health Plan policy
  • Serve as member advocate and facilitator to resolve barriers to care
  • Provide education and coordination of community resource referrals
  • Develop, implement, maintain, and evaluate individual care plans
  • Collaborate with Care Coordination Team to improve healthcare delivery
  • Monitor hospital encounters of assigned members and provide necessary education/interventions
  • Refer clinical decisions beyond authority to manager for review
  • Participate in the Health Plan Quality Improvement process
  • Utilize Health Plan software for service requests and documentation
  • Maintain time-sensitive documentation for regulatory compliance
  • Coordinate services for members following Health Plan policy
  • Perform other duties as assigned
Requirements
  • *
    * Education:

    ** Current Nursing License (RN) required
  • *
    * Experience:

    ** 2+ years of clinical experience as an RN
  • *
    * Experience:

    ** 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • ** Licenses &

    Certifications:

    ** Current Nursing License (RN) required
