• Case Manager II

The Case Manager II reports directly to the Program Coordinator of the U.S.VETS program to which the Case Manager is assigned. The CM II is responsible for providing guidance to veterans to achieve their highest level of independence. Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the selection of services to optimally meet each individual client’s needs.

Responsibilities
  • Assess veterans referred to the program for problems, needs or barriers in areas such as Housing, Employment, Education/Training, Financial, Sobriety Support/Addiction Treatment, Legal, Family/Social, and Medical/Psychiatric
  • Coordinate care with VA personnel and other community partners on behalf of clients for eligibility documentation and mental and physical health services
  • Facilitate client intake process and compile client files ensuring all program eligibility documentation is complete
  • Collaborate with clients to complete Individual Action Plans (IAP)
  • Maintain confidential client files in a timely manner, ensuring all forms are the most recent versions and compliant with organization standards
  • Meet with clients as specified by program requirements and document progress notes in Data/Assessment/Plan (DAP) format
  • Review and document updates for IAPs monthly
  • Complete timely discharge summaries
  • Conduct home visits (at project‑based units or scattered sites) to provide case management services and ensure health and welfare of clients
  • Assist in quality management activities, data collection, and preparing reports
  • Facilitate client life skills classes
  • Provide specialized case management services and life skills groups/classes to veteran subpopulations as applicable (e.g., female veterans, disabled veterans, Iraq/Afghanistan veterans)
  • Conduct random drug and alcohol screenings of clients
  • Maintain confidentiality in compliance with HIPAA standards
  • Complete accurate and timely data entry into the HMIS system and other required internal and external databases
  • Document client referrals for services
  • Attend scheduled team meetings and trainings
  • Maintain and submit time sheets, mileage logs, client expenses, and other required documentation accurately and in a timely fashion
  • May drive company vehicle for client transportation or other work‑related activities
  • Perform other duties as required
Program‑Specific Duties Transitional Housing Clients
  • Provide case management services that result in clients transitioning to permanent housing
  • Ensure clients are accounted for daily through contact, bed check, and/or daily sign‑in logs
  • Monitor program rent/participant fee compliance to assist clients in prioritizing housing
  • Locate housing units in the community that meet clients’ needs for scattered‑site transitional housing such as the Transition In Place (TIP) Program
Permanent Housing Clients
  • Provide case management services that result in clients obtaining and maintaining permanent housing
  • Locate housing units in the community that meet clients’ needs for scattered‑site permanent housing
  • For programs that serve families with dependent children, ensure that children and youth are enrolled in school and connected to appropriate community services, including early childhood education programs
Workforce Clients
  • Assist with Workforce Program Assessment and Enrollment processes
  • Work with clients to develop an Individual Employment Development Plan, assess, alleviate, and reduce barriers to employment, and assist with short‑ and long‑term employment goals including education and training goals
  • Provide individualized assistance in developing cover letters, resumes, and obtaining supportive services
  • Facilitate groups or classes such as Job Placement, Job Retention, Employment Barriers, Computer Skills, Motivation, Money Management
