• Evaluates and treats hospice patients in a Home Setting
• Communicates with families, physicians, and other health care professionals
• Maintains daily, thorough documentation of services in the EMR
• Possesses own equipment including smart devices and blood pressure monitor

• Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy
• Education and/or
  
  Experience:

• Actively Licensed as a FL Physical Therapist

PRN therapist – part of an on‑call pool, working on a per diem basis.
