Job SummaryThe Family Support Facilitator provides direct services to clients identified by the funding source. This includes in-home and community support with clients, resources, and community partners, with the goal of providing support services to moderate and high‑risk families, to reduce the likelihood of a future incidence of maltreatment and involvement with the child welfare system. The role requires training in Solutions‑Based Casework and Family Team Conferencing, additional assessments, and the application of strength‑based work, family engagement, and risk management principles.
Documentation is maintained routinely as part of the work with each family.
- Learn and apply principles of Solutions‑Based Casework, Family Team Conferencing, and related assessments.
- Attend and participate in meetings with internal and external stakeholders, including community providers, partners, and families.
- Utilize creative and flexible problem‑solving to meet family needs.
- Review intake information of referred clients.
- Ensure community services and treatments are delivered and customized to meet individual family needs.
- Assist families with transportation solutions, including connecting with community transit options.
- Provide culturally competent service delivery to each family and client.
- Submit accurate documentation on services provided and family functioning, including entry into the FSFN database as required.
- Manage case load weekly or as required by program with supervisor.
- Build alliances and partnerships with community providers.
- Adhere to all applicable policies and procedures of One Hope United.
- Complete required trainings as stipulated by One Hope United and the lead Community‑Based Care agency.
- Bachelor’s degree required and a minimum of two years of experience.
- State of Florida Child Welfare Certification (CWCM) preferred, but not required.
- Reliable transportation, valid driver’s license, and acceptable driving record.
- Ability to work in a culturally diverse environment.
- Bi‑lingual in Spanish or Creole preferred.
- Up to 20 paid vacation days, 6 paid personal days, 5 paid sick days, and one mental health day per year.

The position requires visual and auditory acuity within normal ranges, with correction if needed. Adequate manual dexterity is required to operate a computer and office equipment. The employee may occasionally need to move items up to 50 pounds. Work occurs in varied residential environments and may involve unpredictable clients. The employee travels by automobile, may encounter changing weather conditions, and can be required to enter and exit vehicles or buildings without assistance.
Frequent automobile travel may occur within the local and regional area, with occasional travel outside the local area and overnight stays.
