• Partner with the Medical Director and supervising physician to deliver comprehensive primary care services
• Provide high‑quality, culturally competent care in an outpatient community health setting
• Collaborate with an interdisciplinary team to ensure exceptional patient outcomes
• Manage and support chronic disease care through patient education and clinical interventions
• Perform a wide range of services, including:
• Routine check‑ups and illness treatment
• Chronic disease management
• Medication management
• Annual physicals
• Women’s health exams, including Pap smears

• Active, unrestricted Nurse Practitioner or Physician Assistant license in Texas
• Board Certification in Family Medicine
• Master’s degree in a relevant field
• Recent, hands‑on experience in Primary Care or Family Medicine
• Current BLS certification (American Heart Association or American Red Cross)
• Spanish language skills are a strong plus
