OVERVIEW:

A respected and mission-driven skilled nursing provider is seeking a knowledgeable and experienced MDS Coordinator / PDPM Nurse (RN or LVN) to join our leadership team. This is for a nurse looking to step into a strategic role focused on care quality, compliance, and reimbursement optimization.

About the Role:

You’ll be responsible for leading the Minimum Data Set (MDS) assessment and care planning process to ensure accurate, complete, and timely submissions. You’ll collaborate closely with the interdisciplinary team to support the facility’s success under the Patient-Driven Payment Model (PDPM).

Key Responsibilities:

Coordinate and oversee all MDS assessments in accordance with state and federal regulations

Ensure timely and accurate completion of MDS documentation

Optimize reimbursement through accurate RAI process and PDPM utilization

Collaborate with nursing, therapy, and administrative staff to ensure cohesive resident care plans

Monitor quality indicators and participate in QAPI initiatives

Educate staff on documentation best practices related to MDS and PDPMMaintain compliance with CMS regulations, including audits and survey readiness

Preferred Qualifications:

RN or LVN licensure in the state of Texas (required)
Minimum of 2 years’ MDS experience in a skilled nursing facility

Strong knowledge of PDPM, RUGs, and Medicare/Medicaid regulations

Excellent attention to detail and communication skills

Experience with Point Click Care  or similar EHR systems preferred
