BASTROP LOST PINES NURSING AND REHABILITATION CENTER in Bastrop, TX is seeking an experienced MDS Coordinator / PDPM Nurse (RN) to join our leadership team. You will lead MDS assessments, care planning, and ensure timely submissions under PDPM.
The role collaborates with interdisciplinary teams, monitors quality metrics, and maintains CMS compliance. Texas RN license and at least 2 years MDS in SNF required; PDPM/RUGs knowledge a must; RAC-CT is a plus.
