In Home Healthcare LVN - Low Acuity

We are seeking compassionate Licensed Vocational Nurses (LVNs) to provide skilled nursing care in a home setting for pediatric patients requiring feeding tube management and medication administration.

• Delivering and documenting patient care in accordance with physician orders and care plans
• Assessing patient condition, recognizing changes, and reporting findings promptly
• Educating patients and families on disease management, self-care, and prevention
• Maintaining confidentiality and adhering to professional and regulatory standards
• Participating in ongoing education and skill development
• Ensuring infection control and safety standards are met

• Active, unrestricted LPN/LVN license in the practicing state
• Current CPR certification
• TB test within the past 12 months
• Reliable transportation; six months of clinical experience preferred
• Physical ability to lift up to 50 pounds and perform patient repositioning
• Effective communication, organization, and problem-solving skills
