In Home Healthcare LVN - Low Acuity

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting, focusing on pediatric patients with needs such as feeding tubes and medication administration.

• Delivering and documenting nursing care according to physician orders and care plans.
• Monitoring and assessing patient conditions, reporting changes promptly.
• Implementing and updating care plans, taking appropriate actions to achieve outcomes.
• Educating patients and families on disease management and self-care strategies.
• Maintaining compliance with policies, regulations, and safety standards.
• Participating in ongoing education and ensuring infection control measures.

• Current, active LVN license in the practicing state.
• CPR certification (with hands-on component).
• TB screening within the last 12 months.
• Reliable transportation and ability to perform physical tasks such as lifting and repositioning patients.
• Preferred:
  Six months of recent clinical experience, home health experience is a plus.

• Attention to detail, effective communication, and time management.
• Problem-solving skills and ability to work independently in varied environments.
• Physical capability to perform necessary tasks in a home care setting.
