IDD Service Coordinator

The Service Coordinator provides services to children and adults with developmental disabilities based on the Person‑Centered Thinking model. The role includes assessment of needs, development of service plans, facilitation of service linkages, monitoring of service effectiveness, crisis intervention and prevention supports, and delivery of advocacy services. The coordinator works with families and agency personnel, respecting cultural differences, and empowers individuals to achieve life goals and live effectively within their families and communities.

The Service Coordinator will monitor services provided to individuals in accordance with standards and billing guidelines established by Medicaid, HHSC, MCOs, and BTCS, as applied to individual cases. Responsibilities include advocating and linking individuals to job readiness or employment activities through the Employment First project; participating in interagency planning and CRCG meetings to bridge gaps and avoid duplication of services; conducting comprehensive assessments;

developing service plans; making referrals and linkages; monitoring, advocating and documenting services; and interacting with individuals, families and service providers.

Position requires frequent initiative and independent judgment, assertive professional intervention, in‑person contact with individuals, families and service providers, minimal supervision, travel within the service area, and a flexible schedule.

• $1.00/hr for 2 years of previous experience
• Up to $4.00/hr for tenure with BTCS
• $1.00/hr for LCSW‑A, LPC‑A, Advanced Licensure
• $0.50/hr for LCDC

• Associate degree or higher in social, behavioral, or human service field (e.g., psychology, social work, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, criminal justice) or
• High school diploma or Texas‑recognized equivalent with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities.
• Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.

• At least one year of experience working with individuals with intellectual/developmental disabilities.
• Bachelor’s degree.
• Demonstrated ability to use good clinical judgment, work well with others and operate effectively in a fast‑paced setting.
• Demonstrated computer and documentation skills.
• Knowledge of appropriate community resources.
• Bilingual (English/Spanish/ASL).
• Experience with Medicaid programs.
