Harmony Cares is seeking a Nurse Practitioner to perform annual risk assessments in patients’ homes or via telehealth, within the NP scope of practice as delegated by the Collaborative Physician.
You will conduct in-home visits, take histories and vitals, determine diagnoses, document visits to NCQA standards, place referrals, and communicate with PCP, caregivers, and nurses while maintaining confidentiality.
