Job Purpose or Mission

Conducts thorough assessment of the patient's nutritional status, creates/provides personalized nutrition treatment plans and in-depth counseling/education that is essential in helping patients make sustainable lifestyle changes. Performs the job for the age population served, as defined in the outpatient clinic scope of service.

Functions
  • Performs nutritional assessments.
    • Reviews and interprets the order/referral to ensure accuracy and that proper nutrition guidelines are followed.
    • Interviews patients to obtain information regarding food habits and preferences for guidance in planning the patient's diet.
    • Recommends appropriate oral diet based on considerations of lab values.
    • Recommends protein, fiber, and carbohydrate requirements with considerations of risk factors.
  • Develops and implements a plan of care based on assessment of nutritional needs in correlation with other health related issues/problems.
    • Calculates medical nutrition therapy order and provides nutrition counseling for all patients who require intervention.
    • Maintains accurate record of medical nutrition therapy instructions performed.
    • Creates customized nutrition treatment plan for all medical nutrition therapy patients.
