• Must have prior nursing home experience
• Master's Degree Required
• Ability to multitask

• Plans and directs a comprehensive social services program which provides support services for residents and families
• Coordinates all discharges to ensure facility census is kept at its maximum capacity
• Participates in an interdisciplinary team approach to better serve the needs of the residents
• Identifies and meets the psychosocial needs of residents by providing therapeutic interventions
• Performs an initial evaluation/assessment of each resident’s psychosocial needs to be placed in the medical record
• Develops social service component of the residents’ plan of care identifying specific needs, goals and approaches
• Completes timely documentation of social service interventions, assessments and progress notes in the resident’s medical record per facility policy and state and federal guidelines
• Acts as a liaison/advocate between residents, families, outside agencies, and the Administrator to ensure that the resident’s rights are maintained
• Submit continued stay requests to LDH
• Complete Admission paperwork
