As RN Navigator I, you will be the liaison and communicator with patients, caregivers, healthcare providers, multidisciplinary team members, post‑acute care facilities, and third‑party payers.

The RN Navigator I facilitates alternative care options, collaborates with the multidisciplinary team, and assists with discharge planning, ensuring smooth transitions and quality, cost‑effective outcomes across the continuum of care.

• Provide continuity of care by coordinating transitions between settings and maintaining patient‑centered care plans.
• Manage high‑risk, complex patient care to minimize readmission.
• ate with physicians and the multidisciplinary team to communicate and implement updated coordinated care plans.
• Prepare, execute, and reinforce post‑discharge care plans.
• Identify barriers to care and elicit changes in processes for patients navigating the continuum of care.
• Review processes with leadership to improve the clinical experience for patients and referring physicians.
• Adapt communication style to specific patient populations, respecting privacy and ensuring clear explanation of services.

All candidates must meet minimum requirements described below. Preferred qualifications are indicated in parentheses.

• Required:
  
  Registered Nurse Diploma.
• Preferred:
  Associate’s or Bachelor’s degree in Nursing.

Required:

2 years of nursing experience.

• Required:
  
  Current RN license in the state of practice.
• Required:
  
  Basic Life Support (BLS) from the American Heart Association.
• Preferred:
  Certification in a clinical specialty area.

• Computer skills for data entry and retrieval of patient information.
• Effective verbal and written communication skills to present information clearly and professionally.
• Proficiency with Windows‑style applications and various software packages.
• Excellent conflict resolution skills.

Essential functions include frequent exertion of 10 to 20 pounds, occasionally up to 100 pounds, and the ability to stoop, bend, reach, and grab. Reasonable accommodations may be made for individuals with disabilities.
