We’re seeking a compassionate, patient-centered Family Nurse Practitioner (FNP) to join our growing multidisciplinary team. As a Family Nurse Practitioner, you will:

Perform comprehensive health assessments and physical examinations.

Diagnose and manage acute and chronic medical conditions using evidence-based clinical guidelines.

Develop individualized treatment plans that promote long-term health and wellness.

Order, interpret, and review laboratory tests and diagnostic studies.

Prescribe medications and other treatments within your scope of practice.

Provide preventive care, annual wellness visits, routine physicals, and age-appropriate screenings.

Manage chronic conditions including:

Hypertension Diabetes Hyperlipidemia Asthma  and COPDThyroid disorders

Obesity

Collaborate closely with behavioral health providers to coordinate care for patients with co-occurring medical and mental health conditions.

Educate patients on disease prevention, nutrition, medication adherence, and healthy lifestyle choices.

Coordinate referrals with specialty providers and community resources when appropriate.

Maintain timely, accurate, and compliant documentation within the electronic medical record.

Participate in interdisciplinary team meetings and quality improvement initiatives.

Provide care through both in-person and telehealth appointments, as appropriate.

Provide comprehensive medical care for patients receiving behavioral health and substance use disorder (SUD) treatment.

Perform medical evaluations and ongoing health monitoring for individuals with co-occurring mental health and substance use disorders.

Identify, assess, and manage common medical conditions associated with substance use, including withdrawal risks, infectious diseases, nutritional deficiencies, and chronic medical illnesses.

Collaborate with psychiatrists, addiction medicine providers, therapists, nurses, case managers, and recovery support staff to develop coordinated treatment plans.

Support Medication-Assisted Treatment (MAT) services by providing medical evaluations, monitoring patient progress, and coordinating care with MAT providers when appropriate.

Educate patients on harm reduction strategies, relapse prevention, medication adherence, tobacco cessation, and healthy lifestyle modifications.

Coordinate referrals to specialty care, detoxification programs, inpatient treatment, community recovery resources, and other healthcare services as appropriate

Qualifications

Master’s or Doctoral degree from an accredited Family Nurse Practitioner program.

Current, unrestricted Family Nurse Practitioner license.

National Board Certification (AANP or ANCC).Current DEA registration (or eligibility to obtain).Current Basic Life Support (BLS) certification.

Excellent clinical assessment, diagnostic, and decision-making skills.

Outstanding communication, interpersonal, and patient education abilities.

Preferred Two or more years of outpatient primary care experience.

Experience practicing in an integrated primary care or behavioral health setting.

Experience caring for patients with co-occurring medical and mental health conditions.

Telehealth experience.

Proficiency with electronic medical record (EMR) systems.

Experience working in an integrated behavioral health, addiction medicine, or substance use disorder treatment setting.

Knowledge of evidence-based practices for substance use disorders, co-occurring disorders, and recovery-oriented care.

Experience collaborating within multidisciplinary treatment teams serving individuals with complex behavioral health needs.

Familiarity with Medication-Assisted Treatment (MAT), including coordination of care for patients receiving medications such as buprenorphine or extended-release naltrexone (preferred but not required).
