Seeking a qualified Home Health RN in Baton Rouge, Louisiana for a 13‑week travel assignment. The role involves direct nursing care in patients' homes, ensuring comfort, stability, and effective management of medical conditions.

• One year of recent experience as a Registered Nurse, including a minimum of six months in a Home Health setting.
• Proficient in OASIS documentation.
• Valid RN license in compliance with state regulations.
• BLS certification (AHA/ARC).

• Home Health Nursing Certification (RN‑BC).
• Any other certifications and licenses required for this position.

• Assess patient conditions and develop individualized care plans.
• Administer medications, treatments, and wound care while monitoring responses.
• Provide patient and caregiver education on medication management and self‑care techniques.
• Monitor for changes, report concerns, and adjust care plans as needed.
• Coordinate with physicians, therapists, and other healthcare professionals for seamless transitions.
• Document patient care and interventions accurately and in compliance with regulatory standards.
• Ensure compliance with infection control, safety, and home care best practices.
• Support families and caregivers with guidance and emotional support.
• Maintain communication with case managers and interdisciplinary teams.
• Complete charting and documentation within visit and productivity goals.
• Perform other duties within scope of practice.
