United Health Group's Heart of Hospice, part of LHC Group, is seeking a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management and supportive care for hospice patients in the Baton Rouge area.
You will conduct face-to-face visits, coordinate plans of care with the Hospice Medical Director and other physicians, educate patients and families, and document care plans and patient responses, while delivering compassionate, high-quality care.
