Job Location: Battle Creek, MI
Working Hours: Monday–Friday, 8:00 AM–4:30 PM
Department: Clinical Care / PACE Program

Position Summary
The Clinical Care Registered Nurse (CCRN) provides direct nursing care to PACE participants, independently applying advanced clinical judgment to manage acute and chronic conditions. This role includes clinic and home-based care, triaging emergent needs, and coordinating with Primary Care Providers and interdisciplinary teams. The CCRN ensures high-quality care while supporting the frail, elderly, and long-term care populations. Key Responsibilities

• Perform clinical triage and manage emergent patient conditions.
• Monitor acute and chronic medical conditions in collaboration with PCPs.
• Implement individualized care plans for clinic and home visits.
• Administer medications, treatments, and procedures as ordered.
• Conduct regular home visits for wound care, lab draws, assessments, and skilled nursing tasks.
• Communicate with caregivers, staff, and external agencies as needed.
• Maintain documentation using Tru Chart EMR.

Required Qualifications & Skills

• Associate Degree in Nursing required; BSN preferred.
• Active Michigan RN license.
• Minimum 1 year experience with frail, elderly, or long-term care populations.
• Home care and wound care experience preferred.
• Current BLS certification (AHA).
• Reliable transportation, valid MI driver’s license, and car insurance.
• Fully vaccinated or willing to comply with vaccination requirements.
• Strong assessment, triage, and clinical decision-making skills.
