Home Health RN

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to provide care in Battle Creek, Michigan and surrounding areas. The position offers an ongoing schedule of approximately 32 hours per week with 8‑hour shifts.

• Lead and collaborate with the care team to manage and deliver patient care plans.
• Perform clinical assessments and provide quality patient care.
• Maintain accurate and timely charting.
• Drive to patient locations, spending approximately 20% of the day on transportation.
• Self‑manage time and schedule to meet critical deadlines.
• Use tablets, mobile phones, and EMR software for patient documentation.

• RN license in the state of practice.
• Graduate of an approved practical nursing school.
• At least one year of RN experience and clinical competence.
• Valid driver’s license and ability to commute to patient locations.
• Strong charting and time‑management skills.
• Proficiency with tablets, mobile phones, and EMR software.
