Clinical Care Registered Nurse
The Clinical Care Registered Nurse (CCRN) role provides direct nursing care to participants. This role works independently in the application of advanced nursing knowledge and skills. The CCRN is able to manage complex clinical situations. This role collaborates with both licensed and non-licensed personnel. Key responsibilities include triaging emergent situations and remaining independent in the application of advanced nursing knowledge. Monitor acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
Utilize care plans independently to provide direct care in clinic and home settings. Administer treatments, procedures, and medications as prescribed to participants. Make home visits regularly to provide care and treatment as ordered by the PCP. This may include wound care, lab draws, nurse assessments, and other nursing tasks. Qualifications include a bachelor's degree preferred and an associate's degree required.
Minimum of one year of experience working with the frail, elderly, or long-term care population is necessary. Home care experience is preferred, and one year of wound care experience is also preferred. Candidates must demonstrate core values of compassion, integrity, teamwork, and excellence. The work environment includes visible presence while working and is subject to frequent interruptions. Communication with staff, participants, caregivers, and other organizations is necessary.
Work may extend beyond normal working hours, including weekends and holidays.
