- Under general supervision, manage the care delivery for patients throughout the length of stay to achieve quality outcomes and timely discharge, including:
- Utilizing the nursing process of assessment, planning, diagnosing, implementation and evaluation.
- Coordinating care provided by other professionals.
- Delegating selected acts, tasks or functions within the scope of practice and the abilities of aides.
- Providing physical care appropriate to the patient’s acuity and complexity.
- Documenting and communicating patient response during each tour of duty.
- Employees providing direct patient care must demonstrate competencies specific to the population served.
- Standards of Practice:
- The registered nurse collects comprehensive data pertinent to the patient’s health or situation.
- Assesses the patient’s physical and developmental needs.
- Analyzes assessment data to identify appropriate patient needs.
- Documents nursing history and age‑appropriate physical/emotional needs and strengths for assigned patients.
- Reviews vital signs to assess patient condition and detect deviations from normal.
- The registered nurse analyzes the assessment data to determine patient problems or issues.
- Uses the assessment data to identify patient problems.
- Confirms problems with patient and family when appropriate.
- Documents expected outcomes and plan of care related to the problems using clinical pathways and outcome teaching records.
- The registered nurse identifies expected outcomes for a plan individualized to the patient.
- Utilizes patient/family input when defining outcomes, incorporating patient values, ethical and cultural considerations.
- Coordinates discharge planning for outcomes not achieved utilizing the continuum of care available.
- The registered nurse develops a plan that prescribes interventions to attain expected outcomes.
- Incorporates clinical pathways into the plan when appropriate.
- Develops individualized interventions with patient input when appropriate.
- Considers data from other members of the healthcare team when developing the plan of care.
- The registered nurse implements the identified plan.
- Implements appropriate interventions.
- Administers prescribed medication and treatment in accordance with approved nursing techniques and policies.
- Utilizes evidence‑based interventions specific to the patient’s plan.
- Maintains awareness of comfort and safety needs of the patient.
- Initiates a patient education plan according to individualized needs and coordinates implementation.
- Manages the care plan by organizing and delegating patient care interventions to be implemented, evaluating effectiveness of care given by self and others.
- The registered nurse evaluates progress toward attainment of the expected outcomes.
- Evaluates effectiveness of the planned interventions and the patient’s response toward achievement of expected outcomes.
- Documents patient response toward the expected outcomes.
- The registered nurse collects comprehensive data pertinent to the patient’s health or situation.
- Standards of Professional Performance:
- The registered nurse systematically evaluates the quality and effectiveness of nursing practice.
- Participates in quality improvement activities.
- Uses results of quality improvement activities to initiate changes in nursing practice.
- The registered nurse attains knowledge and competency that reflects current nursing practice.
