This position includes variable shifts, weekends and holidays and may involve inpatient and ED work.

Responsible for moving patients from admission through discharge without disruption to their care through the process of assessment, planning, implementation, coordination, monitoring and evaluation of patient caseload. Ensures appropriate care is based on patient needs and the hospital’s capabilities. Serves as an advanced clinical resource to patients, families, and staff. Works collaboratively with the interdisciplinary team to provide a continuum of comprehensive cost-effective care and monitors outcomes as a process of continuous improvement.

Qualifications
  • Beginning March 31, 2014 all new hires will possess BSN upon hire.
  • Master’s degree preferred.
  • 3 years of experience in an acute care hospital setting required.
  • Licensed Registered Nurse in good standing with the State of Michigan.
  • Case Management Certification preferred.
  • Ability to use word processing, spreadsheets, keyboard, presentation programs and other relevant software.
  • Ability to handle multiple priorities in a stressful environment.
  • Effective verbal and written communication with all levels of healthcare providers.
  • Ability to communicate in a manner that patients and families find understandable, collaborative and supportive.
  • Demonstrates critical thinking, decision making and problem solving abilities.
  • Effectively negotiates, influences and follows up on situations/issues in a timely, appropriate manner.
  • Ability to assess, prioritize, plan, organize, monitor and evaluate patient needs.
  • Works independently and is self‑motivated.
  • Utilizes effective negotiation and conflict resolution skills.
  • Physical demands include walking, standing, stooping, bending, sitting and some light lifting.
  • Ensures early assessment for patients at risk for post‑hospitalization care and services.
  • Manages and documents patient progress according to procedure.
  • Provides ongoing assessment and maintains contact with patients during care; rounds daily on all assigned patients.
  • Identifies readmissions, reasons and interventions needed to prevent further readmissions and communicates plans to the multidisciplinary team.
  • Ensures smooth transition of care between services.
  • Drives multidisciplinary team rounds and documents clear transitional planning.
  • Enacts transitional plans to move the patient along the care continuum and facilitates post‑discharge needs.
  • Acts as liaison between patients, physicians, ancillary and community services throughout the entire patient experience.
  • Develops, implements, coordinates and communicates the plan of care encompassing the acute phase through transition out of acute care.
  • Maintains strong collegial relationships with physicians, nursing teams and leaders.
  • Coordinates care using Pathways or Plan of Care and undertakes ongoing development and revision of these plans.
  • Participates actively in assigned groups and committees.
  • Uses community and outpatient resources to support care needs after discharge.
  • Coordinates appropriate discharge plans ensuring length of stay is appropriate and achieves quality, clinical and financial outcomes.
  • Evaluates outcomes related to the Case Management process including length of stay, readmission reports, patient satisfaction and financial variances.
  • Tracks and trends all outlier length of stay data to reduce outliers.
House Manager Duties
  • Coordinating and overseeing hospital operations; provides clinical and administrative direction in absence of Unit Leaders.
  • Resolving crises and conflicts; analyzing, assessing and intervening on problems requiring immediate attention.
  • Investigating problems and complaints from patients, visitors, physicians and staff; implementing appropriate action and follow‑up.
  • Conducting initial assessment of atypical events and initiating appropriate action to ensure patient stabilization.
  • Assessing safety concerns and taking action as appropriate.
  • Coordinating admissions, transfers and patient placement.
  • Monitoring hospital capacity and working with Staffing Office to ensure appropriate staff placement.
