LPN Charge Nurse

Calhoun County Medical Care Facility seeks an LPN Charge Nurse to deliver compassionate, resident-centered care in our skilled nursing, rehab, and memory care units. In this role, you will coordinate and oversee nursing care on your assigned unit, provide direct clinical care, administer medications and treatments, and monitor resident conditions. You will collaborate with an interdisciplinary team to develop and implement individualized care plans, support CNAs and other staff, and ensure compliance with safety and regulatory standards.

Responsibilities

• Provide direct nursing care to residents in long-term care, rehab, and memory care units
• Serve as charge nurse for assigned unit, coordinating resident care and staff assignments
• Administer medications and treatments per physician orders and facility protocols
• Monitor resident status, perform assessments, and document accurately in the medical record
• Collaborate with RNs, CNAs, therapists, and providers on care plans and interventions
• Educate residents and families on care plans, conditions, and treatments
• Ensure compliance with infection control, safety, and regulatory standards
• Support, mentor, and guide CNAs and other support staff during shifts
• Participate in care conferences and quality improvement initiatives
• Respond promptly to changes in resident condition and medical emergencies
