Part-Time | 48-64 hours per pay period | Monday to Friday | 8 a.m. to 4:30 p.m.

Pay range: $41.00/hr. to $48.00/hr. (hourly non-exempt) based on qualifications, experience, and location.

As a Speech Language Pathologist (SLP), you'll be at the heart of our community, providing compassionate and personalized therapy services to clients in their homes. Whether developing individualized treatment plans, enhancing communication and swallowing abilities, or collaborating with an interdisciplinary team, you'll play a key role in helping clients achieve their highest level of independence and quality of life.

As a Speech Language Pathologist, you will:

  • Provide Direct Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs.
  • Maintain Accurate Client Records and Billing – Document all services accurately and promptly using Vivie's EMR system. Demonstrate an understanding of all payer sources and authorization requirements. Complete and document supervisory visits with HHAs as per regulatory guidelines to ensure cohesive and compliant care.
  • Develop and Support Follow-Up Programs – Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes. Make referrals to continuation programs as necessary, ensuring clients receive the appropriate follow-up.
  • Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.
  • Ensure Regulatory Compliance – Complete and document supervisory visits with RA/CNAs in accordance with state and regulatory guidelines.
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

This job also requires:

  • Current Speech Language Pathologist (SLP) license in the state of Minnesota.
  • CPR certification within 6 months of hire.
  • Valid driver's license and access to an insured personal vehicle.
  • Ability to pass state mandated background check.
  • Strong clinical skills in speech therapy rehabilitation, skilled therapy services, and knowledge of evidence-based speech and language therapy practices.
  • Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members.
  • Physical capability to perform job duties.
  • Willingness to float across regions as needed.
  • Experience in home health, long-term care, or rehabilitation settings preferred.
