Social Service Assistant

We are seeking a compassionate and dedicated Social Service Assistant to join our team at Carriage House Nursing and Rehab in Bay City, Michigan. As a Social Service Assistant, you will play a vital role in supporting our residents’ physical, emotional, and social well-being by providing assistance with daily living activities, facilitating social events, and promoting a positive living environment.

• Assist residents with daily living activities such as bathing, dressing, and grooming
• Provide emotional support and companionship to residents
• Facilitate social events and activities to promote resident engagement and socialization
• Maintain accurate records of resident activities and progress
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Assist with meal preparation and distribution
• Perform light housekeeping duties and maintain a clean and safe living environment
