* What You’ll Do:
*** Deliver skilled nursing care and oversee pain and symptom management for hospice patients.
* Collaborate with physicians, chaplains, social workers, and other team members to develop and implement care plans.
* Educate patients and their families on the disease process, hospice philosophy, and available services.
* Provide emotional support and guide patients and families through the end-of-life journey.
* Maintain timely and accurate documentation in compliance with agency, state, and federal regulations.
* Participate in case conferences and on-call duties as needed.## Qualifications###

* What We’re Looking For:
*** Active RN license in the state of Michigan.
* Minimum of one (1) year of nursing experience, preferably in hospice or palliative care.
* Compassionate communicator with strong interpersonal skills.
* Ability to travel within assigned coverage areas; reliable transportation, a valid driver’s license, and auto insurance are required.
* Dedication to providing dignified, patient-centered care.###
