The Social Services Discharge Planner assists in providing social services to residents and their families to assist them in dealing with the impact of illness and extended care placement. This position participates in resident care management and ensures that needs are met through Social Service programming and appropriate referrals to outside agencies and community resources.

• Starting Wage: $30.15 per hour

• Assist in meeting the psychosocial needs of each resident and their families in coping with the effects of illness, disability, treatment, or stay in the facility.
• Coordinate services with other disciplines within the facility to enable the resident and families to promote their own well‑being.
• Maintain a working knowledge of residents’ rights.
• Perform timely documentation of Social Service interventions and assessments in resident charts.
• Participate in resident care management, including resident care plan conferences and developing multidisciplinary plans of care.
• Inform residents and/or responsible parties of change of roommate(s) and room changes.
• Assist with admissions, discharges, and MDS testing.

• Degree in Practical Nursing, Social Work, or related human services field (Sociology, Special Education, Rehabilitation Counseling, Psychology, etc.).
• License to practice as a Practical Nurse in Michigan – LPN License.
• 1+ years of supervised social work / case management experience in a health care setting working directly with individuals.
• Excellent customer service skills with a commitment to delivering excellent, passionate, innovative care.
• Team player dedicated to supporting team members and getting the job done.
