Responsibilities:

• Provide direct patient care to residents in a skilled nursing and rehabilitation setting

• Assess patients' physical and emotional needs, develop individualized care plans, and implement interventions to achieve optimal health outcomes

• Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care

• Monitor and report changes in patients' conditions, and maintain accurate and timely records

• Educate patients and their families on disease management, self-care techniques, and available resources

• Participate in quality improvement initiatives to enhance the overall quality of care

Requirements:

• Current Registered Nurse (RN) licensure in the State of Michigan

• Ability to work effectively in a fast-paced environment, with a focus on delivering exceptional patient care

• Strong communication, teamwork, and problem-solving skills
