About the Role:

As a Licensed Practical Nurse at Hampton Nursing and Rehab, you will:

• Provide routine and special care to our residents, including vital signs, treatments, and medications

• Administer injections, oral medications, and other treatments as prescribed by physicians

• Develop and implement individualized care plans in collaboration with our interdisciplinary team

• Monitor and report changes in resident condition to the nursing team and physicians

• Participate in quality improvement initiatives to enhance resident outcomes and satisfaction

• Work closely with other healthcare professionals to ensure comprehensive care

Responsibilities:

• Provide direct patient care to residents in various settings, including skilled nursing facilities and rehabilitation centers

• Perform routine tasks, such as taking vital signs, measuring and recording vital statistics, and preparing resident rooms

• Administer medications, injections, and treatments as prescribed by physicians and in accordance with facility policies

• Develop and maintain accurate and detailed records of patient care, treatments, and medications

• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care

• Participate in quality improvement initiatives and peer review processes to enhance resident outcomes and satisfaction

Requirements:

• Current LPN licensure in the state of Michigan

• Minimum of insert minimum years of experience years of experience as a Licensed Practical Nurse

• High school diploma or equivalent required; diploma or associate's degree in nursing or related field preferred

• Ability to lift, transfer, and reposition residents with assistance as needed

• Strong communication, organizational, and interpersonal skills

• Ability to work in a fast-paced environment with frequent interruptions
