About the Role:
As a Licensed Practical Nurse at Hampton Nursing and Rehab, you will:
Provide routine and special care to our residents, including vital signs, treatments, and medications
Administer injections, oral medications, and other treatments as prescribed by physicians
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Monitor and report changes in resident condition to the nursing team and physicians
Participate in quality improvement initiatives to enhance resident outcomes and satisfaction
Work closely with other healthcare professionals to ensure comprehensive care
Responsibilities:
Provide direct patient care to residents in various settings, including skilled nursing facilities and rehabilitation centers
Perform routine tasks, such as taking vital signs, measuring and recording vital statistics, and preparing resident rooms
Administer medications, injections, and treatments as prescribed by physicians and in accordance with facility policies
Develop and maintain accurate and detailed records of patient care, treatments, and medications
Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
Participate in quality improvement initiatives and peer review processes to enhance resident outcomes and satisfaction
Requirements:
Current LPN licensure in the state of Michigan
Minimum of insert minimum years of experience years of experience as a Licensed Practical Nurse
High school diploma or equivalent required; diploma or associate's degree in nursing or related field preferred
Ability to lift, transfer, and reposition residents with assistance as needed
Strong communication, organizational, and interpersonal skills
Ability to work in a fast-paced environment with frequent interruptions
