As a RN MDS Coordinator
, you will be responsible for coordinating the minimum data set (MDS) process, ensuring accurate and timely completion of assessments, and conducting care conferences to develop individualized care plans. You will work closely with our nursing staff, physicians, and other healthcare professionals to ensure seamless communication and collaboration.

• Coordinate the MDS process, including conducting and documenting resident assessments, care conferences, and developing individualized care plans
• Ensure accurate and timely completion of MDS assessments, care plans, and care conferences
• Collaborate with nursing staff, physicians, and other healthcare professionals to develop and implement care plans
• Provide education and support to nursing staff on MDS processes and care planning
• Maintain accurate and up-to-date records and reports
• Identify and address potential care issues and concerns, escalating as necessary

• Current RN licensure in the state of Michigan
• Strong communication and organizational skills
• Ability to work independently and as part of a team
• Excellent problem-solving and critical thinking skills
• Familiarity with MDS processes and care planning principles
