Social Worker, Crisis Counselor

Transitions Care is seeking a compassionate LMSW Social Worker to join our hospice team. In this role, you'll provide psychosocial assessments, counseling, and resource coordination to patients with serious or terminal illness and their families. You'll collaborate closely with an interdisciplinary team to support symptom management, advance care planning, and end-of-life decision making. Responsibilities include care planning, crisis intervention, grief support, and advocacy to promote dignity, comfort, and quality of life across home and facility-based settings.

Responsibilities

• Provide psychosocial assessments for hospice patients and families.
• Deliver counseling, emotional support, and crisis intervention.
• Develop and implement individualized care plans with the IDT.
• Educate patients and families on hospice philosophy and resources.
• Coordinate community resources, benefits, and support services.
• Advocate for patient rights, preferences, and advance care planning.
• Document assessments, interventions, and outcomes in the EMR.
• Participate in IDT meetings and collaborate with nurses, physicians, and chaplains.
• Support bereavement and grief processes for families.
• Maintain compliance with hospice, state, and federal regulations.
