Social Worker (Bachelors in Social Work) - Part-time - Behavioral Health – Bay Minette, AL

Horizon Health Corporation is seeking a Social Worker (Part-time) for our behavioral health program at Mobile Infirmary and North Baldwin Infirmary in Bay Minette, AL. The Case Manager I is responsible for providing high quality patient care, displaying good skills in assessment, guidance and discharge planning. The Social Worker assists patients and their families in understanding and adjusting to diagnoses and potential treatments.

Social Worker can also help identify resources during treatment and after discharge. The Social Worker effectively participates as a member of the multidisciplinary team and the unit community relations team.

Responsibilities include:

• Implementing a treatment plan of care for patients based on a thorough assessment
• Engaging in timely, efficient discharge planning
• Participating as an active member of the interdisciplinary and treatment teams
• Conducting individual, group and education psychotherapy sessions
• Providing group and individual therapies on weekends as directed by supervisor
• Submitting timely and accurate CQI+ outcome data on a weekly basis

Qualifications:

• Bachelor's degree required
• 0-1 years preferred
• Provisional State license in Social Work (LCSW-A) required
• Valid Driver's License and/or ability to commute to work required
