the role

The Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. The manager coordinates and supervises an interdisciplinary team of staff to assure continuity of high‑quality care to home health patients assigned to the team’s area in accordance with physician‑prescribed plans of care and all applicable state and federal laws and regulations.

Primary Responsibilities
  • Provides clinical services within the scope of practice, as defined by state laws governing nursing, in accordance with the plan of care and in coordination with other members of the health‑care team.
  • Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and plans start of care visits.
  • Coordinates determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
  • Ensures patient needs are continually assessed and care rendered is individualized, appropriate, reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
  • Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations as appropriate.
  • Is able to function in any home situation regardless of a client’s age, race, creed, color, sex, disability, or financial condition.
Required Qualifications
  • Current unrestricted RN licensure in state of practice.
  • Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
Preferred Qualifications
  • Current CPR certification or ability to complete within 90 days of hire.
  • Home care experience.
Pay & Benefits

Pay is based on factors including local labor markets, education, work experience, and certifications. The salary range for this role is $38.80 to $58.20 annually for full‑time employment. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and a 401(k) contribution. Benefits are subject to eligibility requirements.
