Job Title:

Lead Social Worker

Role Overview s a Lead Social Worker at Hudson Regional Health, you will serve as a critical link between clinical innovation and compassionate patient advocacy. You are responsible for steering the psychosocial aspects of the patient journey, ensuring that individuals and their families are supported from admission through a seamless transition back into the community. By blending clinical expertise with a deep understanding of the continuum of care, you will help navigate complex medical and social landscapes to optimize patient outcomes.

Key Responsibilities
  • Assessment & Intervention: Conduct comprehensive psychosocial assessments to identify barriers to care and develop personalized intervention plans.
  • Counseling & Advocacy: Provide high-level clinical counseling to patients and families, offering emotional support and crisis intervention during acute medical episodes.
  • Care Coordination: Facilitate the movement of patients through the healthcare continuum by collaborating with multidisciplinary teams to ensure efficient and safe discharge planning.
  • Regulatory Navigation: Apply expert knowledge of Medicare/Medicaid regulations and JCAHO (Joint Commission) guidelines to ensure all care transitions meet stringent compliance and quality standards.
  • Resource Management: Act as a liaison to community resources, ensuring patients have access to the necessary post-acute support systems, including home health, rehabilitation, or outpatient mental health services.
  • Education: Educate patients and their support networks on disease management, healthcare rights, and the available community-based social services.
Qualifications & Requirements
  • Education: Bachelor’s degree in Social Work or a related field.
  • Experience: 2–4 years of related clinical experience in a healthcare or hospital setting.
  • Licensure: Must be a New Jersey Licensed Clinical Social Worker (LCSW).
  • Communication: Exceptional ability to communicate clearly in English, both orally and in writing, to facilitate complex discussions between medical staff and families.
  • Technical Knowledge: Proven proficiency in discharge planning, JCAHO guidelines for continuity of care, and Medicare/Medicaid regulatory frameworks.
  • Preferred

    Skills:

    Previous experience in a psychiatric or behavioral health setting is highly preferred and considered a significant plus.
