Continuity of Care Coordinator – Hospice
Position SummaryVNA Health Group is seeking a Continuity of Care Coordinator to support after-hours hospice operations, including evenings, nights, weekends, and holidays. This role serves as a key partner to the nursing team by coordinating patient care needs, managing communication between care teams, supporting field staff, and ensuring continuity of care across shifts.

- Support after-hours hospice field staff and clinical teams
- Manage non-clinical patient, family, facility, and staff calls
- Coordinate visits, scheduling, and follow-up activities
- Ensure timely and accurate documentation
- Support communication and handoffs between after-hours and daytime teams
- Escalate urgent issues appropriately
- Monitor vendor performance and service delivery
- Assist with onboarding and training of new staff
- High School Diploma required;
Associate's or Bachelor's degree preferred - Minimum 2 years of hospice experience required
- After-hours hospice experience preferred
- Strong communication, organizational, and customer service skills
- Knowledge of hospice regulations and reimbursement requirements
- Valid driver's license required

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $20.00 to USD $28.00.
