This is a full-time, on-site role for a Physical Therapist located in Baytown, TX. The Physical Therapist will be responsible for evaluating patient needs, developing personalized treatment plans, and implementing therapeutic interventions to improve patient mobility, strength, and functionality. Additional responsibilities include patient education, documentation of progress, collaborating with interdisciplinary teams, and ensuring compliance with care standards and regulations. The role requires fostering supportive relationships with patients and their families to help them achieve their healthcare goals.

• Expertise in Physical Therapy techniques, therapeutic intervention planning, and rehabilitation strategies
• Strong patient evaluation, assessment, and documentation skills
• Proficiency in collaboration and communication with interdisciplinary healthcare teams
• Ability to educate patients and their families about therapeutic exercises and care plans
• State licensure as a Physical Therapist and relevant certifications
• Compassionate, patient-focused approach with excellent organizational skills
• Experience with home health or hospice care is a plus
• Ability to comply with regulatory guidelines and maintain patient confidentiality
