Long Term Acute Care (LTAC) Registered Nurse (RN) position in an inpatient setting. The role involves providing comprehensive nursing care to patients in a specialized facility, emphasizing patient safety, individualized care planning, and promoting recovery and rehabilitation.

Experience:
BLS Certification required prior to first shift; ACLS certification required prior to first shift (CIRH only); minimum two (2) years bedside clinical experience; 0 years of experience required. - Schedule:
Nights shift, 12 hours per shift, minimum of 36 hours per week. - Performs a nursing assessment on all patients within 8 hours of admission, forming the basis for individualized care plans.
- Identifies teaching needs and collaborates with patients, families, and the interdisciplinary team to establish goals.
- Provides timely updates on patient information and status for team conferences.
- Responsible for accurate, succinct, and timely documentation of assessments, interventions, and patient responses in the medical record in accordance with policy.
- Participates in patient rehabilitation programs on the assigned unit.
- Manages direct patient care using the nursing process: assessing, planning, implementing, and evaluating patient care with a focus on education and safety.
- Initiates discharge planning from the day of admission and provides patient teaching in anticipation of discharge.
- Communicates pertinent patient status changes to ensure continuity of care and safety, including informing physicians and team members.
- Provides direction, training, and supervision of ancillary nursing personnel, delegating appropriately.
- Ensures personal and environmental safety following hospital and nursing policies.
- Contributes positively to the team environment by providing calm direction and reassurance to staff, patients, and families.
- Maintains communication with management regarding supplies, equipment, and clinical training needs.
- Provides cost-effective care while promoting quality patient outcomes, monitoring supplies and charges.
- Assumes charge responsibility following orientation as requested.
- Identifies opportunities for professional growth and pursues ongoing development through continuing education and self-study.
