As a Hospice RN Case Manager, you will:- Provide skilled nursing assessment and care in patients’ homes and communities
- Develop and implement individualized plans of care
- Assess patient and family needs and coordinate appropriate services
- Collaborate with the interdisciplinary group (IDG)
- Communicate effectively with patients, families, physicians, and team members
- Monitor medications, supplies, and equipment for appropriate utilization
- Document care accurately and maintain compliance with hospice regulations and standards
- Assist with On call requirements
- Current RN license in the State of Ohio
- Hospice, home health, or community nursing experience preferred
- Compassionate, organized, proactive, flexible, and dependable
- Strong clinical judgment and communication skills
- Ability to work independently while being a collaborative team member
- A genuine desire to provide exceptional end-of-life care
- NURSING : 1 year (Required)
- CPR Certification (Required)
- RN License (Required)
- Beachwood, OH and surrounding communities willingness to travel:
- King David Campus
- Surrounding communities (limited off campus).
