Position Overview
Case Manager – Children’s Program serving clients in Delaware. The role coordinates case management and linkages to community programs and services, coaches and counsels individuals and families within their caseload, and provides crisis intervention.
Responsibilities- Coordinate case management and linkages to community programs and services.
- Coach and counsel individuals and families within their caseload.
- Provide crisis intervention.
- Deliver case sessions primarily in the community (client homes, schools, etc.).
- Work in an office environment or community setting.
- May be required to work evenings and/or weekends to meet client needs.
- Provide on‑call coverage as needed.
- Bachelor’s degree in psychology, sociology or social work.
- At least one year of experience working with children or adolescents.
- Valid driver’s license and clean driving record.
- Able to work Monday‑Friday with an on‑call rotation.
