• Provide direct patient care to residents in our skilled nursing facility, including administering medications, performing treatments, and documenting patient information
• Collaborate with interdisciplinary team members to develop and implement individualized care plans that meet the unique needs of each resident
• Monitor and report changes in resident condition, including any concerns or issues that may impact their overall well-being
• Demonstrate a strong understanding of clinical skills, policies, and procedures to ensure the delivery of safe and effective care
• Maintain accurate and up-to-date records of patient care, treatment, and progress
