Center Well Home Health is seeking a dedicated LPN/LVN to join our home-based care team. Reporting to the Clinical Manager, you will provide skilled nursing services in patients' homes, following individualized care plans and coordinating with an RN and physicians.
You will educate patients and families, document visits and outcomes, and participate in discharge planning. The role requires a valid nursing license, CPR, a driver’s license and reliable transportation; schedule is 40 hours weekly.
