Role:
Licensed Practical Nurse (LPN) providing in-home care for Pediatric and/or Adult patients.
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services according to prescribed orders during assignments.
- Monitor signs and symptoms, identifying physiological and psychosocial changes.
- Collect, analyze, and document patient data and nursing diagnoses.
- Current LPN/LVN license in the practice state.
- Valid TB or Chest X-Ray and BLS certification.
- Preferred: at least one year of professional nursing experience.
- Strong assessment and clinical skills.
- Ability to interpret health data and adapt patient care accordingly.
- Effective communication and documentation skills.
