Position Summary
The PAS Case Manager oversees ancillary programs such as Star+PLUS, Primary Home Care, Family Care, Community-Based Alternatives Program, MDCP, and Private Duty/Personal Assistance/Sitter Services
. This role ensures quality care for patients and families while maintaining compliance with agency policies and scope of practice.
Key Responsibilities
- Supervise all Personal Assistance Services/Community Care Program employees in planning and coordinating patient care per physician’s plan and state program guidelines.
- Assist with billing for ancillary program patients and ensure accuracy in scheduling and office coordination.
- Collaborate with HR to maintain complete and compliant staff files.
- Ensure adherence to regulatory guidelines and agency policies.
- Conduct supervisory visits and document notes promptly.
- Communicate effectively with State program and MCO caseworkers regarding client issues.
- Hire, train, and in‑service quality staff; track unstaffed hours and hiring efforts.
- Participate in meetings, case conferences, and budget planning.
- Maintain confidentiality and perform other duties as assigned.
Skills & Attributes
- Strong knowledge of home care rules and regulations.
- Excellent organizational, time management, and interpersonal skills.
- Ability to work independently in a fast‑paced environment.
- Reliable transportation, valid driver’s license, and auto liability insurance required.
- Proficient in MS Word, Excel, and agency‑specific software.
- Bilingual (English/Spanish) preferred.
Experience & Education
- PAS Program management experience preferred.
- Minimum of 2 years supervisory experience or 2 years of college coursework.
- Strong communication and writing skills.
- Ability to type 35 wpm with 80% accuracy.
