Salary: $55,000 – $80,000.
Schedule:

Qualifications- Master’s degree from an accredited social work school or related field.
- Valid license, registration, or permit to practice in the state(s) of agency operation, if required.
- Medicare/Medicaid/Insurance specialty preferred.
- At least one year of professional social work experience in a healthcare setting (post‑graduate); 2+ years preferred.
- Experience or education in grief counseling preferred.
- Discharge planning experience preferred.
- Assist core members of the pre‑hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient’s health, and support the development of a person‑centered plan of care.
- Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
- Assess social, spiritual, and emotional needs to estimate the patient’s and caregiver’s capacity and coping potential.
- Prepare the patient and family for change and provide education on advance directives and advanced care planning.
- Utilize available resources, including chosen family, hospice, and community agencies, to help the patient and family live well within the limits of the illness.
- Support the bereavement program.
- Provide discharge planning related to changes of level‑of‑care or community placement/locations of care.
- Licensed to practice as a clinical social worker in the state of agency operation, if required.
- Licensed driver who can travel to all business locations.
