Summary/Objective

The LVN Inpatient Coordinator provides a variety of patient care activities for a designated patient caseload, while supporting the provider(s) in facilitating patient care to enhance the quality of clinical outcomes and patient satisfaction.

Job Duties

  • Reviews daily assigned Physician(s) procedure schedule and hospital rounds report to ensure all appropriate information is available for the assigned Physician(s).
  • Responsible for providing assistance to the physician with examinations, procedures, and patient education, as well as performing other patient care activities as assigned.
  • Actively monitors and documents information pertinent to care coordination and the progress of patients to ensure all assigned patients are seen timely.
  • Performs nursing skills effectively according to established standards of care, as well as demonstrates interpersonal competencies such as problem solving, negotiation, and conflict management.
  • Interacts with physician practices, pharmacies, and drug reps while maintain a professional level of conduct at all times.
  • Participates in in-service programs and other self-directed learning activities to promote practice, as well as actively participates in performance improvement activities.
Required Education And Experience
  • Associate Degree
  • 1-3 years of related experience
Required License/Certifications

Current LVN license in the State of Texas
