Hospice RN Case Manager – Full-Time Beaumont, TX

We're looking for a compassionate Registered Nurse Case Manager to join our Beaumont team.
What You'll Do:
- Manage a caseload of hospice patients throughout the Beaumont area.
- Complete comprehensive nursing assessments.
- Develop and update individualized plans of care.
- Coordinate care with physicians and the interdisciplinary team.
- Educate patients and families on disease progression and symptom management.
- Document visits using our EMR system.
- Participate in IDT meetings.
Qualifications:
- Texas RN License.
- One year of nursing experience.
- Hospice or Home Health experience preferred.
- Strong assessment and communication skills.
- Reliable transportation.
