Position: Collaborate Care Case Manager OBI Salary Range: $40,407 - $44,482 / Annually

JOB PURPOSE:

The Collaborative Care Case Manager is responsible for carrying a caseload of dually diagnosed intellectual and developmental disabilities/behavioral health consumers who have multiple, complex needs and who would benefit from collaborative case management services to prevent potential crisis situations. The manager will provide consultation, training, and technical assistance for individuals with intellectual and developmental disabilities and for the staff who support them, in accordance with agency policies, operating procedures, and applicable state and federal statutes and regulations.

The role also includes supporting caregivers and community providers to ensure successful diversion from institutional placement and transition to community settings.

EDUCATION AND EXPERIENCE

REQUIRED:

Bachelor's or Advanced Degree from an accredited college or university with a major in a social, behavioral, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.

Yes. This position requires periodic local travel with occasional out‑of‑area travel for trainings and meetings, which may involve use of the agency vehicle or personal car with mileage reimbursement.

Knowledge, Skills & Abilities
  • Good spelling and grammar skills.
  • Ability to write progress notes, assessment reports, brief mental status exams, memos, letters, various complex reports, and other documents with acceptable spelling and grammar.
  • Proficiency with technology, including Microsoft Office and Google applications.
  • Ability to input data into computers, perform calculations, complete paperwork, and produce reports.

  • May be required to interact with consumers, referral sources, outside agencies, and family members to determine needs and actions to be taken.
  • Current state‑of‑Texas driver license or, if living in another state, a valid license from that state.
  • Liability insurance is required if the employee operates a personal vehicle on Center property or for Center business. The vehicle must be insurable by Center’s liability carrier.
  • Must complete all training requirements in the month in which they are due.
  • May be required to provide on‑call duties.
  • Availability of a personal vehicle for business use may be required.
  • Work will occur in office, community, and client homes, including shelters and temporary housing.
  • Must submit to and pass a pre‑employment drug test.
  • Must successfully pass all background screens as required by the state.
  • Must be physically able to conduct home visits, including walking on uneven surfaces, climbing stairs, and standing for long periods if required.
Essential Tasks & Standards
  • Ensures coordination of support meetings and crisis plan development for individuals served.
  • Provides home visits and visits to day and vocational settings as needed.
  • Develops cross‑systems crisis prevention and intervention plans, completes intakes and assessments, and creates outcome plans for respite or inpatient hospital admissions/discharges.
  • Reviews records of individuals with IDD in various settings to assess gaps in care and advocate on their behalf.
  • Promotes understanding and self‑management of behavioral, environmental, and psychiatric challenges for clients, service coordinators, and caregivers.
  • Identifies and documents interventions to reduce crises for persons at risk of or transitioning from institutionalization.
  • Participates in systemic consultation.
  • Recommends referrals to skills training as appropriate.
  • Submits required documentation of data collection.
  • Maintains linkages and relationships with community partners.

  • Conforms to the Center’s Compliance Program in all aspects of employment.
