Role Description

This is a full-time, on-site Social Worker role located in Beaver Dam, WI. The Social Worker will provide emotional support, counseling services, and advocacy for residents and their families, while also coordinating care plans and facilitating access to community resources. Day-to-day responsibilities include conducting assessments, creating individualized service plans, providing crisis intervention and support, and maintaining accurate documentation in alignment with organizational policies.

Collaboration with an interdisciplinary team to ensure holistic care is also a key component of this role.

Qualifications
  • Comprehensive knowledge of social work principles, methods, and practices
  • Strong interpersonal and communication skills; ability to build rapport with residents, families, and staff
  • Proficient in case management, counseling, and providing emotional support
  • Organized and detail-oriented to effectively prepare and maintain thorough documentation
  • Understanding of community resources and how to connect residents with appropriate services and support
  • Master’s Degree in Social Work or a related field is strongly preferred; licensure or certification as a Social Worker is required
  • Experience in a healthcare or long‑term care environment is a plus
  • Ability to work collaboratively with interdisciplinary teams to develop and implement care plans
